I am a newbie so here goes. I assume it is necessary to obtain written approval when using a trademark logo. If my assumptions are correct then:
1. Where would one begin obtaining such permission?
2. Are there any examples of consent forms on the internet?
3. Are royalties typically paid out when using a logo in a retail product?
4. Are my assumtptions correct? Specifically other company logos and band logos.
