"Specimen image" when renewing service mark

I am electronically filing the Section 8 Affadavit (Declaration of Use of Mark in Commerce Under Section 8), since it has been 5 years since my trademark was registered. What I have is a service mark, which consists simply of the name of my web site, including the .com. There is no artwork or logo; the original registration simply had a typed drawing which was the name spelled out and nothing else.

Where I am stuck on the Section 8 Declaration form is at the Specimen Image File. What do they want here? Do they want simply another black-and-white typed drawing consisting of just the name typed out? Or do they want an example of the service mark in use, i.e., a screen shot of my web site, or what?

What sort of image should I submit and what exactly should I type in to the next box, "Describe what the specimen submitted consists of:"

That's exactly what they are looking for.  Be sure your use qualifies as a "use in commerce".

I'm not sure what you're asking in the first part of the quoted language, but as for the description box, describe what you're submitting, e.g., "website advertising services" or "website constituting point of sale materials," as appropriate.
