Non-English specification

Greetings!

It is said, that you should file a translation of specification, if the application was filed in non-English language. It is also said, that together with the translation you should also file a statement, that it (translation) is correct and nothing was added/changed.
But I can't find, how should this statement look like? Should it be just a pdf-file with a phrase like "I am the translator and I certify that the translation is correct" or...? And, by the way, do we need to notarize it?

Zaralun:  A signed document, captioned “translator’s certification”, with a simple statement like:
I {name} certify that I am competent to translate from {the foreign language} into the English language and that the accompanying translation of {foreign application number} for {title} filed {date} is a true and accurate translation thereof.” Has worked for me.
