Motion to Amend Involved Application

Anyone have an example of one of these or can lead me to one?  Basically a motion to amend dates of first use in interstate commerce...

I'm not sure I'm following the question.  There is a an eTeas form for this you can use, and the law is summarized in TMEP § 903.04.
