Is it possible to notify a TQAS at the USPTO that and Examiner is issuing unacceptable Office Actions?

Yes, but it can be difficult to get the relevant QAS' information.  You could try calling the USPTO helpdesk to request contact information for the relevant QAS.

Might want to start with the SPE first.  They are in the best position to correct an Examiner who is acting inappropriately, and by going to the QAS, you are effectively going over the SPE's head (bad if you have not consulted them first).

Assuming you haven't had a conversation with the SPE already, cold calling a QAS will get you referred to the SPE.

If you've talked with the SPE I would actually call the TC director's office. Any complaint of a quality nature could possibly get pawned off on a TQAS anyway.
