Your post was so full of accurate detail that I presumed you to be asking and not telling.
 
But you are absolutely correct.  In fact, if more than a year is passed the MPEP recommends that you provide the additional information with your request rather than waiting for the PTO to ask for it.

I have never had the opportunity to file a petition to revive.   Does anyone have any information as to how often they actually revive?

I've only done it twice, and it's not been a problem.  I used PTO/SB/64; it's pretty straightforward.
