question regarding registration/correspondence

I work at a small biotech company. I used my employer as correspondence when I applied for a seat in the patent bar exam. Now I passed the exam.
1. Can I change to my home address as correspondence? There are two separate lines in the data sheet form. One is CORRESPONDENCE/BUSINESS NAME, the other is CORRESPONDENCE/BUSINESS ADDRESS. I need to complete all lines. What should I put in the CORRESPONDENCE/BUSINESS NAME line? my name??
2. If I keep my employer as correspondence, can I accept private clients or represent clients other than my current employer?
