Would I have a good shot at passing if studied with an old set of PLI books based off MPEP ed. 8, rev. 2?  Or are the changes with rev. 4 so substantial that i'd be too confused to  answer the knit-picky technical questions on the exam?

You can look at the revision notes (the "blue pages") for the two more recent MPEPs, and then look at the differences.  E8R4 only differs from E8R3 in chapter 2300, which had no changes between R2-R3.  (I think they must not have been able to finish the changes for that one in time, so issued everything else in E8R3 and then made a one-chapter update for E8R4 -- R4 came out pretty soon after R3.)
 
Go to the USPTO MPEP page and look for "MPEP Archive" near the top to get the old versions, including the blue pages section.
