I apologize in advance for the basic questions:
 
Do you only need to take the Patent Bar exam one time?  
 
Is it a state-to-state thing or is it that if you pass it once, you can be a patent agent in any state?

If you pass on the first try, yes.

Registration as a patent agent allows you to practice in patent matters before the United States Patent and Trademark office, so if you are a registered patent agent you can practice anywhere.
