data sheet - business name/address

Hi,
I need to fill out the form - "data sheet - register of patent attorneys and agents", there is space for business address and home address, can I leave the business address blank if I do not want to fill in ?
It says on the form "complete all lines", what should I do ?
If I fill in both address, how do I indicate that I want home address to be published and I want to be contacted through home address?
Any suggestion, solution?

I don't know, but you can always call the OED to ask. Its ph. number is on the PTO website.

Just fill in both with the same address. You want registration-related business mail to be sent to your home address. Therefore, your home address is your business address in this instance.
