I am interested in working at the USPTO. Do they require pre-employment or random substance testing?

Yes.
I don't think they would want to hire people who
use illegal drugs.

You still have to answer questions about previous
drug use, and if you lie and they find out, you will be fired.
Also you are not protected under the ADA. It is
not a disability to decide to start using illegal drugs.
