Could someone tell me the process of assigning a patent.  Is the only basic requirement is the cover page, and assignment agreement?  Thanks

Assigning means tranferring the rights associated with a patent.  That is not something you do at the patent office, but rather is something that occurs between you and the assignee.  It needs to be in writing, but it does not actually have to be recorded at the patent office.
 
What the patent office does is record an assignment.  The patent office is not interested in all of the terms of your agreement.  It is only necessary to document a binding agreement between the parties.  Recordation does protect the assignee so that the assignee should insist on recording with the patent office, but it really is no skin off the nose of the one assigning.
 
For recordation, a cover sheet and an agreement and the 40 dollar recordation fee are what is required.  The details are easily found in the MPEP.

By MPEP I refer to the "Manual of Patent Examining Procedure" which is the practice guide for interacting with the USPTO for patent matters.   There is an online copy at the PTO web site.
