Copyright for entire website

Hi, I have read the circulars, and your page on copyrights, but I am still fuzzy on what I actually need to do to copyright a website. What form(s) do I need to fill out, and what do I need to send them - a print out of the pages, or the sourcecode for the pages? I want images, text, names, etc. all to be copyrighted. I have already put up copyright notification on all pages, but I need to make it official now.
If you can help, it would be MOST appreciated! Thanks!
P.S. Also, say I want to take something off of another website who has a copyright on that stuff. If I get their permission to use it, do I have to display "copyright **** Other Company, Inc." or what? Or do they tell ME what I do? :)
