: : I currently re-created a Work Comp Form from the Dept of Labor. They have 2 copies in Adobe and another in Word created with text boxes. I created mine using a table with form field so our company can put it on our intranet. We are currently submitting it for approval, however we are worried about other companies getting a hold of this form. If we did not create the "original" outline can we still put our company name or logo on it?
